"La baseline est issue des rapports programmatiques des quatre trimestres de 2019 du PNLP. Les nombres des cas enregistrés sont des cas confirmés par les tests de diagnostic rapide. Compte tenu de la forte augmentation de cas enregistrés pour 2019, soit 49402 cas notifiés et également avec 15000 cas notifiés en janvier 2020 qui représente 30% des cas attendus en 2020. Le PNLP a revu ses cibles pour l'année 2021 ;2022 et 2023 pour faire face aux besoins du programme notamment en médicaments antipaludiques, dans la lutte antivectorielle, la prise en charge , dans la surveillance épidémiologique et de renforcement des capacités. Ainsi pour determiner la cible de 2021, Il a été appliqué un taux d’augmentation de 5% sur la valeur de base qui est de 49402. Par conséquent il est estimé un nombre de cas paludisme confirmés de 51872 pour la période de 2021. Cependant en 2022 et 2023 le PNLP estime une diminution de cas de paludisme de 2,86% et 5%. En effet, en tenant compte de la réalisation des activités ciblés telles que : la distribution de masse de MILDA dans les zones à risque en 2022 ; ainsi que pour les refugiés et les migrants de façon continue pendant les trois années de la subvention et la mise en œuvre de la PID de façon graduelle pour 2021, 2022 et 2023 le renforcement de la surveillance et la gestion de gites larvaires. Le PNLP estime une cible de 50316 en 2022 et 47800 en 2023. Un ralentissement de l'augmentation des cas est réaliste car les effets expliquant cette forte montée (en partie la délétion du HRP2 donc retard diagnostic et retard de prise en charge ainsi qu'une qualité de pulvérisation intradomiciliaire diminué par les évènements climatiques)seront adressés par le programme en plus d'une extension de la PID à l'ensemble des quartiers les plus touchées sur financement du gouvernement. Ce rapport permettra de mettre en place les décrets de création ou dextension de ces AMPs. - Les plans de gestion des aires marines protégées (AMPs) au niveau de chaque site du projet sont en cours de finalisation par le cabinet Hydroterra/Cordio. - Le recrutement dun gestionnaire en ressources maritimes a été réalisé par le MUET. - Plusieurs compagnes de sensibilisations ont été réalisées sur la préservation des milieux marins et côtiers pour les communautés de Sagalou et Haramous dans la préfecture de Tadjourah et de Arta - La stratégie et le plan daction sur le genre ont été finalisés. - Élargissement du réseau national dAMP et renforcement de la gestion des AMP au niveau du site - Une étude de référence sur la biodiversité marine et côtière à Djibouti ainsi que les plans de gestion des Aires Marines Protégées ont été élaborées et - Un atelier de validation des études de références et des plans de gestion des AMP a été organisé avec toutes les parties prenantes du projet ; - Une mission de sensibilisation des touristes en matière de protection de la biodiversité marine et côtière a été effectuée et des guides de bonnes pratiques aux pêcheurs vivant sur la zone d’intervention du projet, ont été distribués ; - Des activités de nettoyage dans la zone côtière de Djibouti ont été réalisées. - La réalisation des travaux de construction des infrastructures qui servira de bases pour les éco-gardes a été réalisé sur l’AMP d’Arta Plage/Ghoubet et de Rasali/Sable Blanc ; Activité 1, première réunion du comité de pilotageUne fois que le projet a été approuvé une seconde fois le 25 mars 2021, un comité de pilotage a été créé pour le projet afin de fournir des orientations stratégiques et des orientations en matière de gestion pour la mise en œuvre du projet. Le comité de projet se compose de représentants de toutes les principales parties prenantes et veille à la prise en compte des intérêts du gouvernement, du bureau de pays du PNUD, ainsi que de représentants du secteur public.Sur invitation du Ministère de l’Environnement et du Développement Durable, la première réunion du Comité de Pilotage s’est tenue le 22 avril 2021 au palais du peuple dans le cadre du Projet intitulé « Gestion durable des ressources en eau, des p turages et des périmètres agropastoraux du bassin versant Cheikhetti ». Activité 2, restauration des périmètres agricoles dans la localité de Cheikhetti Dans le cadre de la composante 3 du projet relatif à l’agropastoralisme résistant au climat, des activités portant sur la restauration et aménagement des jardins existants sont prévues. Ainsi, chaque année sur la base des consultations de la communauté du bassin versant de Cheikhetti, des jardins seront réhabilités. Ainsi, dans ce sens, une mission terrain a été effectuée pour consulter les bénéficiaires du projet et les autorités locales afin d’identifier les jardins prioritaires à réhabilités. Ensuite, le consultant en génie civil assisté par l’UGP sera chargé d’élaborer un cahier des charges pour procéder à la réhabilitation de ces derniers. Un rapport détaillé relatif à cette mission a été rédigé par le coordinateur du projet. Une fois que les jardins prioritaires pour la réhabilitation ont été identifiés, les travaux de réhabilitation vont débuter prochainement. Activité 3, lancement de l’évaluation environnementale et sociale des activités du projet Conformément au plan de travail annuel (PTA) validé lors de la première réunion du comité de pilotage, une évaluation environnementale et sociale a été lancée le mardi 21 septembre 2021 en collaboration avec le cabinet ECARE consulting & rechearch qui a obtenu le marché à la suite d’une demande de soumission d’une offre technique et financière auprès de 3 cabinets national capable de réaliser la présente étude. Activité 4, Finalisation de létude de référence du projet Cheikhetti La réalisation d’une étude de base pour le projet Cheikhetti s’avère nécessaire surtout que le projet a été finaliser et ficelé il y’a cinq ans. Cette situation de référence permettra d’établir le point de départ des différents indicateurs (situation avant) afin d’être utilisé pour la comparer avec la situation après la mise en œuvre du projet (situation après). Cela permettra d’apprécier qualitativement la contribution de chaque composante à l’atteinte des objectifs du projet visant à améliorer la résilience des communautés pour faire face aux impacts négatifs du changement climatique et la dégradation des terrains agropastoraux du bassin de Cheikhetti. Activité 5, Construction/réhabilitation des puits communautaire dans le bassin versant de Cheikhetti Dans le cadre de la composante 2 intitulé réhabilitation des sols et reconstitution des aquifères dans le bassin versant de Cheikhetti, une activité correspondante à la construction des 7 nouveaux puits et à la réhabilitation de 4 autres puits existants sont prévues. La phase PPG du présent projet avait permis de pré-évaluer les points deau susceptibles d’être réhabilités ou les nouveaux points deau susceptibles d’être installés avec la participation de la communauté bénéficiaire du projet. Ainsi, du 23 au 25 Novembre 2021 une mission terrain a été effectué afin d’identifier les sites propices à la construction des puits communautaires et de localiser les 4 puits nécessitants la réhabilitation. La DEDD a publié lavis dappel doffres intitulé Travaux de construction, lacquisition des équipements et linstallation dune centrale solaire à Moumina 1 . Pour maximiser la concurrence et assurer une large dissémination, la DEDD a mis en ligne les documents de lavis dappel doffres et a republié lavis dans les éditions pour une semaine encore. - Lépidémie du COVID-19 et le confirmèrent ont impacté considérablement la mise en œuvre du projet. Suivant les discussions sur le Cadre de Performance qui ont eu lieu le 11 mai 2019, les équipes ont adopté l’hypothèse 3, qui prends en compte l’estimation des cas sur la base de la progression trimestrielle entre 2017-2018-2019. En effet, le PR a d’abord considéré la part des cas enregistrés durant le trimestre 1 de l’année 2017 et de l’année 2018, sur l’ensemble des cas enregistrés durant les deux années, soit respectivement 36% et 48%. Il a par la suite calculé la moyenne entre ces deux pourcentages, qui est de 42% et il a appliqué ce pourcentage au nombre de cas enregistré au premier trimestre 2019 (19204) pour obtenir le nombre de cas attendu sur les trois derniers trimestres de 2019, soit 30342. Ainsi, le nombre total de cas attendu en 2019 était de 49546 (19204+30342). Par la suite, il a appliqué le pourcentage de 36% (part du trimestre 1 de l’année 2018) au nombre de cas enregistré au premier trimestre 2019 (19204) pour obtenir 31495 sur les trois derniers trimestres de 2019, soit un total de 50699 cas attendus sur toute l’année 2019 (31945+19204). Ensuite, le PR a calculé la moyenne entre les 49546 et les 50699 cas attendues selon les deux calculs, soit 50122. Ainsi le nombre de cas de paludisme attendu en 2019 est de 50122. Le prorgramme a acquis des réactifs HIV-Could qui seront utilisés sur le geneXpert pour le dépistage des nouveaux-nés de méres séropositives pour l'année 2019 et la mise en place d'un circuit de référence des enfants nés de mères séropositives vers le service pediatrique des différents hopitaux. De plus, nous notons l'existence d'un comité de suivi de PLS-s DSME depuis février 2020 .Ceci permettra de renforcer le suivi des enfants nés séropositifs. Le résultat de cet indicateur sera disponible au mois de juin de chaque année après validation de SPECTRUM de l'ONUSIDA." "Selon le rapport mondial TB 2020 de l'OMS, l'incidence estimé de la tuberculose est de 234 cas pour 100 000 habitants. Elle va évoluer en baisse les prochaines années avec une réduction de 4% par an au cours de la période 2021-2023. Cet indicateur et les cibles sont ceux du PSN TB 2020-2024 (230 en 2021, 221 en 2022 et 212 en 2023). Le pays tend vers les nouveaux objectifs mondiaux en diminuant le taux d'incidence de la Tuberculose. Le PNLT suivra cet indicateur pour voir la tendance de la Tuberculose. Le résultat de cet indicateur sera disponible en Octobre de chaque année lors de la publication du rapport mondial de la Tuberculose." "Ces cibles sont celles du PSN TB 2020-2024. Cet indicateur mesure le taux de mortalité par la tuberculose au sein de la population générale. Source des données: Rapport mondial OMS publié chaque année. Pour diminiuer le taux de mortalité, le PNLT renforcera le suivi des malades pour diminuer tout risque de perdus de vue et augmenter les succès thérapeutique, pour cela les activités suivants seront mis en place: un nouveau système de tracabilité de patients référés par le biais de la flotte téléphonique et un relance systématique des patients irréguliers sera effectif pour récupérer les malades. En outre un dépistage précoce des cas présumés par l'identification des sujets contacts des cas index. Le rapport OMS 2020 rapporte un taux de 25 décès pour 100 000 habitants en 2019 (www.who.int/tb/data). Le PSN vise la réduction du nombre de décès lié à la TB de 11% par an (soit pour 100 000 habitant 20 en 2021, 18 en 2022 et 16 en 2023). Le résultat de cet indicateur sera disponible en Octobre de chaque année lors de la publication du rapport mondial de la Tuberculose." "La baseline est issue des rapports programmatiques des quatre trimestres de 2019 du PNLP. Les nombres des cas enregistrés sont des cas confirmés par les tests de diagnostic rapide. Compte tenu de la forte augmentation de cas enregistrés pour 2019, soit 49402 cas notifiés et également avec 15000 cas notifiés en janvier 2020 qui représente 30% des cas attendus en 2020. Le PNLP a revu ses cibles pour l'année 2021 ;2022 et 2023 pour faire face aux besoins du programme notamment en médicaments antipaludiques, dans la lutte antivectorielle, la prise en charge , dans la surveillance épidémiologique et de renforcement des capacités. Ainsi pour determiner la cible de 2021, Il a été appliqué un taux d’augmentation de 5% sur la valeur de base qui est de 49402. Par conséquent il est estimé un nombre de cas paludisme confirmés de 51872 pour la période de 2021. Cependant en 2022 et 2023 le PNLP estime une diminution de cas de paludisme de 2,86% et 5%. En effet, en tenant compte de la réalisation des activités ciblés telles que : la distribution de masse de MILDA dans les zones à risque en 2022 ; ainsi que pour les refugiés et les migrants de façon continue pendant les trois années de la subvention et la mise en œuvre de la PID de façon graduelle pour 2021, 2022 et 2023 le renforcement de la surveillance et la gestion de gites larvaires. Le PNLP estime une cible de 50316 en 2022 et 47800 en 2023. Un ralentissement de l'augmentation des cas est réaliste car les effets expliquant cette forte montée (en partie la délétion du HRP2 donc retard diagnostic et retard de prise en charge ainsi qu'une qualité de pulvérisation intradomiciliaire diminué par les évènements climatiques)seront adressés par le programme en plus d'une extension de la PID à l'ensemble des quartiers les plus touchées sur financement du gouvernement. " "Actuellement le pays ne dispose que de données du premier Trimestre 2020 sur les cas de décès dus au paludisme. Le PNLP dans le but de collecter cet indicateur a tenu une réunion avec le secrétaire général du ministère de la santé en février 2020 . Ce dernier s'est engagé à faire une note à l'égard des hôpitaux de référence et de 5 CMH en vue de notifier le cas de décès dans le registre d'hospitalisation déjà mis à leur disposition par la DIS. Toutefois le programme prévoit les activités suivantes dans la subvention actuelle : -Tenir une réunion avec les différents directeurs, médecins des hôpitaux et le ministère des biens Waqfs pour la collecte des cas de décès (qui a pour fonction principale de répertorier les cas de décès dans les hôpitaux de référence). -Elargir multisectoriellement en intégrant plusieurs ministères tels que le Waqfs (Affaires musulmanes). Pour fixer les cibles de cet indicateur le PNLP est parti du nombre de cas graves rapporté au deuxième semestre de 2019 qui est de 132 à laquelle un pourcentage a été appliqué (20%). il est attendu : - En 2021 de 26 décès soit 20% des cas graves. - En 2022, il est prévu une baisse du taux de mortalité de 10% des 132 cas soit un nombre de décès attendu de 13 cas . - Et enfin de 5% des 132 en 2023 soit un nombre de décès attendu de 6 cas . Ainsi le programme s'est fixé une cible supérieure au PSN en tenant compte de nombre de cas grave enregistré dans les hôpitaux . Par ailleurs, en T1 2020 17 cas de décès liés au paludisme ont été notifiés dans les hôpitaux de référence ( 7 décès à peltier et 9 à hôpital de balbala et 1 à dar al hanan). Le PNLP determinera la valeur annuelle de référence avec les résultats de 2020. En outre, la colone décès figure desormais dans le registre d'hospitalisation du SMIC que ainsi que le motif d'admission et du diagnostic. Le PNLP estime une cible de 50316 en 2022 et 47800 en 2023. Le PNLP dans le but de collecter cet indicateur a tenu une réunion avec le secrétaire général du ministère de la santé en février 2020 .
